Are you missing a washer in between the rear top mount and the rear shock?
The installation instruction specifies that you separate the rubber on the upside down "V" washer from the stock shock and use that as the bottom washer in between the top mount. If you don't the rubber mount can pull out from the housing. Otherwise -
