I had a faulty before cat O2 in bank 2 recently and this didn't throw any warning lights at all - the only reason I knew it was faulting was because I pretty much scan my car with my clone GT1 every week and my Autoenginuity every other week.
Believe it or not the Autoenginuity picked the fault up first (no dash warning and no drivability issues, no missfire, nothing, it was perfect) - Autoenginuity showed it faulted 156 times and I turned on the datalog function on the autoenginuity and went out for a drive - the data record was more than obvious what was going on - and from what I can make out from the data feedback was it would have been running rather lean whenever thorttle load was over 50%.
Glad I caught this early and certainly very surprised that the knock sensors nor any engine warning lights showed up. What was even more surprising was the fact that the GT1 only showed this faulting twice, but came straight back when the codes were cleared.
Had the O2 replaced straight away and all is now good.
Moral of the story - scan your cars on a regular basis.
